WHAT IS RHINOPLASTY

Rhinoplasty, or nose surgery, is a procedure that allows us to alter the shape and size of the nose to achieve a more harmonious appearance with the rest of the face; however, it can also serve a functional purpose, such as correcting deviations in the nasal septum that make it difficult to breathe through the nose (known as septoplasty).

Are you a candidate for rhinoplasty?

Rhinoplasty may be considered from the age of 18–20 years old, once facial development has been completed. There is no upper age limit, although in patients of an advanced age the a22> assess the general state of health prior to the procedure.

You are a suitable candidate if you find yourself in one or more of the following situations:

  • A hump on the bridge of the nose that disrupts the harmony of the profile
  • A broad, drooping, asymmetrical or flat nasal tip

The procedure is not recommended if you have uncontrolled systemic conditions, blood clotting disorders, active infections in the area, or if your expectations do not align with what the surgery can realistically achieve. During the pre-operative consultation, each case is assessed individually and you will be given full and transparent information about what the procedure can and cannot achieve.

How we perform rhinoplasty at Pedralbes Clinic

The pre-operative consultation

It all begins with an initial consultation, during which the surgeon assesses the overall shape of the nose, listens to the patient’s goals and explains what is technically feasible in each case. A morphological photograph is taken and, where necessary, a CT scan may be requested to plan the procedure in greater detail. This initial consultation is included in the price of the procedure.

The intervention

Rhinoplasty is performed under local or general anaesthesia in Pedralbes Clinic’s own operating theatre. The procedure takes between 1.5 and 3 hours, depending on the complexity of the case. The surgical approach – open, closed or ultrasonic – is determined based on the pre-operative planning.

Once the procedure is complete, a nasal splint is fitted, which the patient will wear for the first 7–10 days. The procedures are carried out on an outpatient basis: the patient arrives in the morning and returns home accompanied by a companion on the same day.

The postoperative period

The post-operative period following rhinoplasty follows a a5> pattern that is predictable and which it is important to be aware of before the procedure:

Days 1 to 7. Swelling and bruising around the eyes are at their worst during the first few days. This is completely normal and to be expected. We recommend taking it easy, applying cold compresses, keeping your head elevated and avoiding strenuous activity. Your nose will be blocked due to the swelling.

Days 7–10. The nasal splint is removed during an outpatient check-up. The swelling begins to subside. The majority of the patients can resume their work activities in office environments at this point in time.

From month 1 to month 3. 70–80% of the final result is already visible. You can take part in sport of low impact. It is important to protect the nose from direct impacts and avoid exposure to the sun without sunscreen.

From month 12 to month 18. The result is considered final once the residual swelling at the tip of the nose has completely subsided. Noses with a hump show results sooner; those requiring work on the tip require more patience.

Dr Joan San Miguel holds a degree in Medicine from the University of Barcelona and in Dentistry from the European University of Madrid.

He holds a PhD from the University of Navarra and is a specialist in facial and maxillofacial surgery, having trained at the prestigious Navarra University Hospital (CHN). A member of the Barcelona Medical Association (COMB) (no. 39345), he is also accredited to practise in Belgium, where he completed a European Fellowship in Cosmetic Facial Surgery at the University Hospital of Brussels (UZ Brussel) under the supervision of Dr Maurice Mommaerts, who is arguably one of the world’s leading facial plastic surgeons.

He is a member of the European Association of Facial Plastic Surgery (EAFPS), the European Association of Cranio-Maxillofacial Surgery (EACMFS) and the Spanish Society of Oral and Maxillofacial Surgery (SECOM), and sits on the board of the Catalan-Balearic Society of Maxillofacial Surgery (SCBCMF).

    What types of nose can we correct?

    Rhinoplasty can address virtually any anatomical variation of the nose. These are the most common procedures performed at our clinic:

    1. Hooked beak or with a hump on the back. The hump on the profile is one of the most common reasons for consultation. Rhinoplasty removes the protrusion and harmonises the bridge of the nose with the overall appearance of the face.
    2. Nose wide. This involves reducing the bridge of the nose, narrowing the base of the nostrils or reshaping the tip, depending on which structures are contributing to the excessive width.

    Rhinoplasty is NOT painful. Don’t worry: during the operation, the patient is under anaesthetic, and after the procedure any discomfort can be easily managed with standard painkillers (paracetamol, ibuprofen). Most patients describe the sensation as more like ‘nasal congestion’ than pain.

    Like any surgical procedure, rhinoplasty, or nose surgery, can carry certain risks. Fortunately, the risks associated with rhinoplasty are few, and complications—both during and after the operation—are very rare when the procedure is performed by a specialist surgeon with experience.

    The recommended minimum age is:

    • Women: 16–17 years old (once facial bone development has completed)
    • Men: 17–18 years old (later development)
    • Maximum age: There is no upper limit if you are in good health

    Depending on the case, we can use local anaesthesia with sedation, deep sedation or general anaesthesia. The majority of our rhinoplasties ultrasonic are performed under local anaesthesia and sedation, which is safer and allows for a faster recovery.

    The duration of a rhinoplasty ranges from 1 to 3 hours, depending on the complexity of the case. A simple rhinoplasty involving correction of a dorsal hump may last 1–1. 5 hours, whereas a complex septorhinoplasty complex may take up to 3 hours.

    Yes, it is often combined with:

    • First 24–48 hours: Relative rest, minimal swelling
    • Days 3–5: You can now lead a relatively normal life, avoiding strenuous activity
    • Day 6–7: Removal of splint. You can return to work (provided it does not involve physical exertion)
    • Week 2: You can do light exercise (walking, yoga)
    • Month 1: A completely normal life; you can do moderate exercise
    • Months 3–6: Result permanent, residual swelling disappears
    • Immediately (with a splint): The basic changes can be seen immediately
    • Day 6–7 (splint removal): 70–80% of the final result is visible
    • Month 1: Appearance significantly improved, swelling reduced by 90%

    Yes, the changes made during rhinoplasty are permanent. The nose will only change as a result of the natural ageing process, just like the rest of the face.

    • Closed rhinoplasty: All incisions are made inside the nose. It leaves no visible external scars. It is our technique of choice in the majority of cases.
    • Open rhinoplasty: A small incision is made in the columella (between the nostrils). This allows better visualisation in cases that are very complex, leaving a scar that is minimal.

    With the closed technique, there are no visible external scars. All the incisions are inside the nose.

    Ultrasonic rhinoplasty is a modern technique for nasal surgery which allows improvement of the appearance of your nose in a more straightforward manner. Instead of using manual tools an ultrasonic scalpel is used which allows a26> ultrasonic scalpel which allows the bone of a shape more precisely, without having to fracture the bone.

    Consequently, recovery time is shorter than with conventional rhinoplasty, as there is very little bruising and significantly less swelling, resulting in more predictable outcomes.

    There may be a temporary loss of smell due to post-operative inflammation, but this will fully recover within the first few weeks.

    It depends on the type of work:

    • Jobs in an office or working from home: 4–5 days
    • Work involving light physical exertion: 7–10 days
    • Jobs involving intense physical exertion: 2–3 weeks

    It is recommended that you do not rest your glasses on the bridge of your nose during the first month. You can use:

    • Contact lenses from the day after
