Rhinoplasty for augmentation is the technique that enables us to increase the volume of the nose, in any of its axes and planes. The most common is the augmentation of the nasal dorsum, and this occurs in the a23> vast majority of individuals originating from East and South Asia, and indigenous Amerindians.

If the deformities are minimal, and the amount of material required is no more than a few tenths of a millilitre, one may opt for the injection of high-density hyaluronate or autologous fat grafting. Although hyaluronate lasts for between 6 months and 1 year, there is some replacement of volume by fibrous tissue as a reaction to the presence of the product. As for autologous fat, there is a reabsorption rate of between 20 and 40% of the amount injected, meaning that 60 to 80% of the fat remains permanently after the first 3–4 months following the injection.

If the required augmentation of the dorsal area involves volumes of between 1 and 3 ml, a rolled auricular cartilage graft with temporal fascia is indicated. The auricular cartilage is harvested from the concha of the auricle. Access is gained via an incision behind the ear, which is perfectly concealed and imperceptible.

Once the cartilage has been removed, it is cut into small 2-mm cubes which are bonded together using a fibrin gel. The graft is covered with temporal fascia to help it retain its shape and ensure a smooth texture, so that no lumps are noticeable. The temporal fascia is removed via an incision on the side of the scalp, which is perfectly concealed and imperceptible. The results of this technique are long-lasting.

If the need for increased stiffness in the back is high and furthermore it requires stiffness to be able to support the weight of the skin of the nose up to the level of the tip and back (to achieve, for example, greater anterior projection and length), the rib cartilage will be used.

The rib cartilage is removed by means of a small incision below the lower edge of the chest. Once the fragment of cartilage has been removed from the breast, it is possible to obtain different pieces to achieve correction or shaping the parameters that require more volume or definition.
