It is essential to carry out a computer simulation

To determine how much of the hump needs to be removed, it is essential to carry out a computer simulation of the desired outcome. This allows the image to be overlaid onto that of the nose prior to surgery. This allows us to determine the order of magnitude of the hump to be removed, the amount of rotation required at the tip and/or the amount of projection reduction to be carried out (if necessary). A frontal simulation can help give an idea of the final result, but it is less predictable, so it is usually used to simulate cases where the nose is crooked.

Rhinoplasty for reduction of the nasal hump

It is usually performed via an incision that is hidden inside the nose (closed rhinoplasty). This means that no visible scar is left after the operation. Once the hump has been removed, the nasal base (the bones on either side of the septum) is usually reshaped to achieve the correct shape at the very top of the nose and prevent an ‘open roof’ deformity.

Although the removal of the hump may seem the part that is easiest of a rhinoplasty, it is the one that requires the most precision and meticulousness. This is to prevent collapse of the cartilages quadrangular (cartilages which lie on either and both sides of the middle part of a33> the nose; if they collapse, the nose may appear crooked).

The alar cartilages are preserved correctly using the Split Hump technique, described by the Dutch surgeon J. Lohuis in 2012. Once we have successfully shaped the upper and middle parts of the nose, we move on to the tip. It is not just a matter of tapering the tip, but of ensuring it looks natural, avoiding excessive angularity, and ensuring it rotates and projects correctly.

The processing time can vary, ranging from 1 hour to 2 hours, although it is important to remember that it is more important than the result is good than doing it quickly, as suggested by some authors with the use of the technique “rinofast”.

To ensure that every detail is properly addressed, it is important to take your time and be thorough. Once the surgery is complete, the nose must be properly cared for by inserting packing (a strip of gauze soaked in an antibiotic ointment is placed inside each nostril to prevent wound infection), applying adhesive strips (known as Steri-Strips, these are applied from the tip towards the bridge, and along the entire bridge, to help the skin adapt to the new shape achieved during the surgery) and fitting a splint (a sort of plastic ‘cast’, which serves to hold the bone in a position determined by the surgeon).

The patient will then be taken to the recovery room, and subsequently to the rehabilitation ward, where they may be accompanied by their family or friends for a period of between 2 and 4 hours, until they have fully regained consciousness. During the post-operative period, antibiotics, painkillers and anti-inflammatories are prescribed until the splint is removed, which is usually 6–7 days after the operation. The dressings are removed 2–3 days after the operation (provided they were applied beforehand).

The basic results are already visible from the seventh day after the operation (the profile of the bridge, the shape of the tip, and any asymmetry); however, the nose is still swollen, and it will take several months for the skin to adjust to the new shape.

It is often necessary to use Steri-Strip-type plasters to help the skin adjust, for at least 12 hours a day, over a period ranging from a few weeks to two or three months, to ensure a better and more stable result. The final result will be visible after one to two years.
