WHAT IS RESURFACING?

We are talking about resurfacing when we refer to all those procedures with which one a14> achieves exfoliation of the skin to a greater or lesser degree, with the aim of achieving an improvement in appearance.

Types of resurfacing

There are three types of resurfacing:

Chemical peels exfoliate the skin through contact with acidic substances, which penetrate varying layers of the skin depending on their strength. For example, glycolic acid peels are the mildest and only exfoliate the stratum corneum and stratum lucidum (the outermost layers). Salicylic acid peels are best performed in a clinic and have a greater penetration capacity, reaching the stratum corneum, lucidum, granulosum and sometimes the spinous layer. This requires more than one session, usually five, each separated by a week. Trichloroacetic acid peels are more potent, reaching the basal layer, and must always be performed by experienced practitioners. In this case, the number of sessions is limited to a maximum of 1 to 3. The most abrasive (but also most effective) type of facial peel is the phenolic acid peel, which is capable of reaching the papillary dermis whilst preserving the basal membrane matrix. In this case, a single session is sufficient, although the post-operative period requires more care than with other peels, with a recovery time of up to 7 days. It is essential that the patient avoids sun exposure during the treatment and for several weeks or months afterwards, as there is a risk of hyperpigmentation and of achieving a poor aesthetic result.

Laser resurfacing exfoliates the different layers of the epidermis using concentrated light (photons). Since 1994, the technique has evolved and a wide variety of different lasers have appeared (and disappeared) from the market. In most cases, a fractional CO₂ or Er:YAG laser is used, both of which have a high penetration power, resulting in very effective outcomes. The effect is similar to that of a first-degree burn, with a recovery period of up to one week, depending on the intensity and type of laser used.

Dermabrasion exfoliates the various layers of the epidermis and the outermost layer of the dermis (the papillary dermis) using a rotating instrument. In this case, dermabrasion is usually recommended for the treatment of so-called ‘barcode wrinkles’ around the mouth. Another indication is acne-related lesions that leave scarring on the cheeks. Dermabrasion requires local anaesthesia and has a recovery period of approximately 7 days. So-called microdermabrasion is a much less aggressive (but much less effective) alternative for the treatment of more superficial wrinkles, and could be compared to the effect of a salicylic acid peel.
